Accessing Your Medical Records

HealthMark Group is our registered Business Associate; they are responsible for fulfilling and completing requests for medical records. After your medical record request is fulfilled, HealthMark Group does not maintain any copy of your record.

Please note, HealthMark Group does not have the ability to schedule, change or cancel appointments, have access to new providers accepted insurance plans or know where your previous physician is now practicing. As noted in the email, Dr. Steinman and Dr. Phillips are joining the U.S. Dermatology Partners network of physicians. To continue to be seen by Dr. Steinman or Dr. Phillips, no action is needed on your part. Simple call to schedule an appointment.

Process Overview:

  1. Patient submits medical record request to HealthMark Group through https://medrelease.healthmark-group.com/360/
  2. HealthMark Group receives request
  3. HealthMark Group processes the medical records requested within 1-2 business days of receipt
  4. After the records are processed, HealthMark Group mails a prepayment invoice to the patient’s address listed on the request
  5. Upon receipt of the invoice, the patient can make payment electronically through our website via credit card. Once payment is applied, the patient will have the option to download their requested medical records directly from HealthMark Group’s website
    • As an alternative to making a credit card payment through HealthMark Group’s website, a check may be mailed to the address listed on the invoice.
  6. Upon receipt of payment, HealthMark Group will immediately make the records available for download through their website

Instructions on Submitting a Request to HealthMark Group:

  1. Access the following website link, https://medrelease.healthmark-group.com/360/
  2. First time users will be required to create an account. Select the button for “Create an account”
  3. Complete the registration information by providing a valid email address and account password
  4. A registration email is sent out to the email address provided
  5. Verify account through registration email
  6. Upon logging into our site, Select the “Submit Request” menu item
  7. Provide all necessary information by completing all required fields and send us your request by selecting “Authorize Release”

  • What will the charge be to receive a copy of my medical records?
    • A fee of $5.00 will be required to cover the cost to process your medical record request. Please note the $5.00 fee covers the cost to receive your records electronically through HealthMark Group’s website. If paper copies are needed or requested, there will be an additional fee for shipping. Please note, these fees are under the allowable rate allowed by the Texas Board of Medical Examiners, which governs what physicians may charge to process medical record requests.
  • What is the expected turnaround?
    • Upon receipt of the incoming request, HealthMark Group will have the records processed within 1-2 business days. After the request has been processed, an invoice will be mailed to the patient’s address listed on the medical record request or found within their demographics. Please allow for standard USPS delivery times. Payment can be made through HealthMark Group’s website, and medical records will be available for download. If the records are directed to another authorized third party, HealthMark Group will deliver the requested records after payment is received.
