Fillers and neuromodulators are two of the most commonly performed cosmetic dermatology treatments. While they work in different ways, both can play an important role in facial rejuvenation.
According to Dr. Lauren Watchmaker of U.S. Dermatology Partners in Phoenix and Scottsdale, Arizona, “Neuromodulators and fillers are highly customizable cosmetic treatments. The best approach depends on each patient’s unique anatomy and aesthetic goals. I create an individualized treatment plan for every patient with the goal of achieving natural, refined results.”
In this blog, Dr. Watchmaker explains the key differences between neuromodulators and fillers, what each treatment is best suited for, and how they can be used alone or together to achieve a patient’s aesthetic goals.
What Are Neuromodulators?
Dr. Watchmaker says, “Neuromodulators are injectable treatments that use botulinum toxin to temporarily reduce targeted muscle contractions that contribute to facial wrinkles. Botox is perhaps the most recognized brand, but other commonly used neuromodulators include Dysport, Xeomin, Jeuveau, and Daxxify.” Neuromodulators are often recommended to reduce the appearance of fine lines and wrinkles on the forehead, between the eyebrows, around the eyes, and on the neck. Neuromodulator results typically last about 3–4 months, although duration can vary depending on the product, dose, and individual patient.
What Is Filler?
Dr. Watchmaker says, “Fillers are most commonly made with hyaluronic acid, a substance naturally found in the skin. When injected, fillers can restore or augment volume, improve facial contours and proportions, and soften certain lines or folds.” Fillers are often recommended to replenish fullness or provide contour in the cheeks, lips, temples, jawline, and the hollows under the eyes. While neuromodulators primarily target dynamic wrinkles caused by muscle movement, fillers are often used to address static lines, folds, or contours that remain visible when the face is at rest. Filler results typically last about 6–12 months or longer, although duration can vary depending on the product, amount injected, treatment area, and individual patient.

Choosing Your Injector Matters
Both neuromodulators and fillers are medical procedures and choosing an appropriately trained and experienced medical professional is essential to minimize the risk of complications. Although rare, fillers can cause serious complications, including vascular occlusion, making injector training and an in-depth knowledge of facial anatomy crucial.
Choosing the Right Treatment
Neuromodulators and fillers can be used individually or in combination. According to Dr. Watchmaker, “There isn’t one treatment plan that is right for everyone. A consultation with an experienced provider can help determine which treatments, if any, are appropriate for you.”
