New Patient Forms

Welcome to U.S. Dermatology Partners Greenville! Now that you have made an appointment for your first office visit, we look forward to meeting you. Please take a few moments to print out our New Patient Forms below and complete them before your visit. You will need to bring them with you.

Please be on time for your office visit. If you arrive late for your appointment, you may be asked to reschedule.


We always try to meet your urgent healthcare needs. There are times when we may not be able to see you as soon as you would like because scheduled appointments take priority. However, we do our best to accommodate true emergencies.

Late Arrivals

We strive to be on schedule for your appointment – and we appreciate you being on time, too! If you arrive late for your appointment, you may be asked to reschedule.


If you are unable to keep your scheduled appointment for any reason, please call our office as soon as possible. This courtesy allows us to serve other patients in need of medical attention. Appointments cancelled or broken without 24 hours advance notice are subject to a possible $50 cancellation charge. Surgery “no show” appointments will be charged a fee of $100.


Payment is required for all services at the time they are rendered. All applicable co-payments, co-insurances and deductibles will be collected at the time of your office visit. We accept checks, cash and Visa, MasterCard, Discover and American Express. There is a $35 charge for all returned checks.

Insurance Referral

If your insurance carrier requires a referral, you must obtain one from your primary care physician in accordance with your insurance company’s guidelines. Bring this referral with you to your office visit. Please familiarize yourself with the number of visits authorized in your referral and the expiration date.

Phone Calls

Phone calls to our office will be returned as soon as possible. Please understand that our primary responsibility is caring for patients who are currently in the office. If the nurse is unavailable when you contact us, your call will be returned as soon as conditions permit.

Lab Results

The doctor will schedule a follow-up appointment to discuss your lab work or pathology results. These results will not be given over the phone.

Prescription Refills

You can request refills of your medications through your pharmacy. The pharmacy will contact us for approval. This process can take 24 to 48 hours, so please don’t allow your medications to run out!

Your doctor may not refill your prescription if he/she thinks it is necessary to see you in person before doing so. In that case, you will need to schedule an appointment with our office. Prescriptions for acute medications are rarely authorized over the phone.
