What to Bring

Welcome to U.S. Dermatology Partners of Austin – Jollyville. Below you will find the information that you need prior to your first appointment.

  • New Patient Packet
  • Minor Consent Form (if patient is a minor)
  • Minor Consent Form – Notary (if you’re unable to accompany your child to the visit)
  • Current insurance card
  • Photo ID
  • List of all current or over-the-counter medications
  • Dermatology history information
  • Copies of previous biopsy reports
  • Photographs of your skin problem
  • List of medications you have used to treat your problems

What to Avoid

Don’t wear makeup if your dermatology problem is on your face. If you must wear makeup, please be prepared to remove it for the examination. You may reapply makeup after your examination.

Don’t wear self-tanning products prior to a complete skin examination. This may change the pigment appearance of your moles and make examination of your moles more difficult.
